	School Registration Form **
	This form is used for the registration of all students and needs to be submitted annually.

	Student Information Card**
	This form needs to be filled out and signed by the parents of all children attending the academy.

	Consent and Release Form **
	We ask that each parent read our policy handbook and then sign at the bottom as well as initial all categories of this form as part of the annual contractual agreement.

	Medical
	This form needs to be completed by physician, physician's assistant or nurse practitioner for all students. Students who are enrolled in the extended day program as well as the Newburgh UPK program need to also have a current medical form on file.

	Daycare Registration Form **
	This form needs to be completely filled out by parents of children in our Infant and Toddler Programs only.
